Nobody plans for a serious illness. However, when it strikes, it can be challenging to navigate the resources needed to care for yourself or your loved ones. Residents of the Bronx can benefit from our holistic approach to care through the Hope Care Management Program.

A comprehensive service, Hope Care Management coordinates medical care, mental health care, and support services to individuals with serious illnesses. Our case managers work to enhance, stabilize and improve our patients’ health outcomes, including reduced hospitalization and emergency room visits, and boost their overall wellbeing and quality of life.

We specialize in at-risk populations who are battling substance use, mental illness, medical diagnosis, and homelessness. To respond to each individual’s specific needs, Samaritan Daytop Village partners with reputable health home service providers serving the Bronx, Queens, Brooklyn, and Manhattan. Services we coordinate include, but are not limited to, primary medical care, specialty medical care, mental health providers, substance use treatment, housing specialists, food and nutrition, and various support services.
